Answer the question as applicable. If yes, mention them and your relationship as well.
Do you wish to work in the United States?
The US Government has lifted the restriction on the work permit of H4 visa holders. Hence, you are allowed to work by meeting the eligibility criteria. Give the answer accordingly.
Do you live with your family presently?
If you live with your family, such as your parents or in-laws, answer honestly.
Which city or town do you live in your native country?
Mention your current residential city or town.
Who will you live with in the United States?
Tell the immigration official that you will be living with your spouse in the US.
Who will support you financially during your period of stay in the US?

Tell the date of birth of your spouse i.e. exact date, month and year.
Which university your spouse graduated from and what is the highest educational degree of your spouse?
